According to Illinois State Police, in Carmi, a Fairfield woman was killed and three others were injured in a head-on crash, which occurred east of Fairfield, at around 9:00am yesterday morning. Killed in the accident was 66-year-old Beverly Johnson, of Fairfield.

According to police, Johnson was driving westbound on Illinois Route 15 in a 2006 Chevrolet Impala when a semi driven by Steve Koontz of Fairfield crossed the centerline and collided head-on with her car. Authorities stated that Koontz was driving behind a pickup truck when that truck started to make a left turn. Koontz was unable to stop and his semi struck the pickup truck from behind, which pushed it off of the road to the right. The semi then skidded into the opposite lane of traffic, colliding with Johnson's car. J.B. Fletcher, of Fairfield, was the driver of the pickup truck.

The Wayne County Coroners Office pronounced Beverly Johnson deceased at the scene of the crash. A passenger in Johnson's car, her 13-year-old grandson was injured and taken to Fairfield Memorial Hospital. Fletcher was taken to Fairfield Memorial Hospital. Koontz was airlifted to Deaconess Hospital in Evansville where he was treated and released.

Route 15 was closed for a considerable time to allow emergency responders access to the scene and for investigators time to determine the cause of the crash, which remains under investigation.

Assisting State Police at the scene of the crash were the Wayne County Sheriffs Office, the Fairfield Rural Fire Department, and the Wayne County Ambulance Service.
